I have been given the following cases to solve in an attmept to understand the topic of 'Patient clinical data analysis' to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and diagnosis and coem up with a treatment plan

Review:

History of the patient is very well taken. The information provided was precise. The investigations that are done are clearly mentioned with dates that helped us to understand about the disease clearly. Diagnosis was clear . The work was very neat.The elog  was very well organized. 

Case 2: CKD

Review:

General examination and history taking was good. Systemic examination is very well presented. Multiple  Investigation report images are kept and are clear   . And the treatment part is more elaborate. It was good overall and easy to understand.

Review:

Symptomatology was well explained. Case was very well presented. 

Multiple images are kept which helps in better understanding and also interesting. 

Presentation was neat and elaborate.

History is very well taken and easy to comprehend. Presentation was elaborate. Detailed information is provided. 

Explanation was good. Diagnosis was clear . Information provided was easy to understand.

A thorough history taking was done. The information was precise. And the case was explained very clearly using images of Investigations and reports. It was good overall. 

Question 4:

CHRONIC KIDNEY DISEASE 

It's a progressive and a chronic condition that affects many aspects of health..

There is gradual loss of kidney function.

SYMPTOMS

Some Patients may be asymptomatic but some may develop symptoms like

  • Fatigue
  • Loss of appetite 
  • Water & electrolyte imbalance. 
  • Sleep disturbances 
  • Shortness of breath.
  • Changes in urinary output 
  • Chest pain
  • Muscle cramps
  • Dry and itchy skin
  • Overall weakness. 
CAUSES OF CKD
  • congenital abnormalities 
  • UTI s
  • Hypertension 
  • Diabetes 
  • Urinary tract obstruction 
INVESTIGATIONS:
  • GFR is used to diagnose CKD.
  • Serum creatinine is estimated 
  • ECG is taken
  • Bone marrow aspiration is done
  • Haemogram 
  • Serum electrophoresis 
  • 2D echo
  • Iron profile.
Treatment
  • Symptomatic treatment is given
  • Healthy diet should be followed 
  • Do not smoke .
  • Limit alcohol.


Question 5:

Making a e log is very helpful for us. We are learning how to diagnose a case and all. By reading different e logs we are understanding how the case should be presented.
